The easiest way to make an appointment would be to get your GP to make a referral addressed to Mr Ausha De Silva. Your GP can provide all the relevant information on your behalf, such as details of all investigations you have had and also any treatment you may have received so far.

However, if you're unclear about whether you need to see Ausha or are considering self-referral, please contact Kim (specialist nurse) at Boulcott Hospital, who will be able to discuss your concerns first.

When you come for your appointment please remember to bring the following:

• Referral letter from GP unless already sent to us.
• Copies of results such as ultrasound scans and blood results etc.
